Process to become client
Call Medicaid Eligibility to confirm your family member is eligible for Medicaid insurance cover.

Once approved for Medicaid coverage, call Personal Care Service (PCS)/ Age & Disability Waiver (A&D) phone number to start the process to get approved for state paid care for your family member. PCS/A&D Waiver Phone Number- 1-800-826-1206
Once you have an appointment set with the Regional RN to discuss your family members’ condition, please call our office to discuss your upcoming appointment and process to begin services and employment.
Process to become paid employee
All employees must pass federal background check, obtain CPR certification, have reliable vehicle and vehicle insurance, Idaho drivers license, current SS card, 2-hour orientation and complete 8-hour online training.
All employees must have a working cell phone with internet connection to provide location when providing care to their family member.
Hearts 4 Seniors in-home care helps all ages and can employ all areas of Idaho residents.
We make this simple by offering ZOOM and online options for individuals not living in the Burley area.

She knows exactly how parents with special needs children benefit from support and assistance on a regular basis.
